Installed EVO-GMT4 and used provided T-harness for use as stand alone unit. Unit programmed according to steps in manual. Disconnected unit and updated to 70.18 using Flash Manager and FLU unit and and decrypted as well. All went well according to Flash Manager. Verified options C1 and D1 were enabled. Reconnected unit to vehicle and blue light flashes each time lock button is pressed, but vehicle never tries to start.

Did you flash the firmware after progamming it in the vehicle? This must be done before...Flash to 70.18, then program it in the vehicle, then dcrypt the immobilizer data at the computer...

Update:

I worked with tech support from the company I purchased the unit. We performed several master resets and performed everything in the necessary order. The tech I spoke with used the same unit on his truck and verified it should be working when it's not. I get a blue light the first two times I press the lock button but no blue light on the third time. When I start the truck with the key the yellow light is lit on the unit. Should there be three blue light flashes and then a yellow when pressing the lock button three times? Tonight I am going to try the lock-unlock-lock option as I do get blue light feedback each time those two buttons are used together. Thanks for the idea and keep them coming if you know anything else.
the blue led should flash on every lock signal sent. Lock unlock Lock is a good idea to try as well. The led's when remote start begins should go red on, blue flashing then yellow on...

Make sure the dark blue wire from the 20 pin is not touching ground!! this will set it into valet mode and prevent it from starting!
